Transitioning from Rehabilitation Into the Home Environment:

Spinal Cord Injury may also include the following secondary conditions: pressure sores, respiratory complications, urinary tract infections, spasticity and scoliosis. Having an in-home team of caregivers who understand how physical and emotional health and family dynamics can be affected is vital to adjusting and transitioning into the home environment. Frequently, individuals may suffer from excessive anger, irritability, depression, and mood swings. Care Plus TLC works within the family environment to establish practical coping mechanisms and practices to help families through the adjustment process.

Our programs incorporate physical care processes and behavioral processes as needed. We help by providing tangible interventions to support, routines and consistency with effective coping skills. Our goal is to help you achieve your highest possible level of independence.

Rehabilitation Window:

Studies show that positive outcomes are directly related to having the right in-home multi-skilled caregiver team. When a spinal cord injury occurs, there is usually swelling of the spinal cord. This may cause changes in virtually every system in the body. After days or weeks, the swelling begins to go down, and people may regain some functioning. Many patients who experience Incomplete Spinal Cord Injuries may recover some function as late as 18 months after the injury.
Our skilled caregiver team is trained to follow individual Plan of Care daily Range of Motion exercises for the BEST POSSIBLE OUTCOME.

In some cases, people with spinal cord injury may regain some functioning years after the injury.

Avoid Compassion Fatigue:

As a family member caregiver, your wellbeing matters too! Your safety and that of your family must be the first priority. You may find that you are suddenly expected to perform treatments way beyond your experience. As survivors are discharged from rehabilitation earlier and with complex injuries, this is becoming a very serious national problem.

Care Plus TLC provides resilience education for family caregivers to learn essential ways to care for themselves and avoid burnout and compassion fatigue.

Secondary Traumatic Stress:

Family caregivers are also at risk of developing a Secondary Traumatic Stress response (STS).

A central feature is that family caregivers frequently experience a high stress level. This response can have physical and mental health consequences, interfering with your family and caregiver responsibilities. STS also makes you more vulnerable to caregiver injury, burnout and compassion fatigue

Choosing the right team is critical to your transition and adjustment home. We advocate for you to involve the right experts for your In-Home Professional Medical Team.

Re-engaging in Life:

Our caregivers enjoy accompanying clients to the movies, errands, family outings, and other appointments. We work closely with your Medical Social Worker to find activities that are FUN and promote independence

Transition / Adjustment:

ADL’s – Activities of Daily Living We help by providing tangible interventions to support routines, consistency and coping skills for spinal injury symptoms. Caregivers are trained using transfer techniques (including but not limited to):*Hoyer/Sarah Lifts *Slide Board and *Gate Belt transfers

Range of Motion Exercises:

Caregivers are skill in conducting daily Range of Motion exercises and apply braces when required. We work closely with the client following the Physician Plan of Care / Physical and Occupational Therapy directives.

Personal Care:

  • Bowel Program

  • Toileting

  • Peg Tube Care

  • Catheter care

  • Wound bandage changes

  • Orthopedic Devices, etc.

Registered Nurse case oversite promote caregiver skill sets and training, which, ensure capabilities in diffusing difficult behavioral challenges.
