Transitioning Home

We are pleased to meet our clients and their families prior to discharge. Our staff will be available to meet your patient and their family and to receive specific training from your clinical team to ensure individual case requirements are met. Family support, education and adjustment processes are often critical components to successful transitions from rehabilitation to the home environment. Our professional RN team honors your desire have your patients receive continued continuity of care.

Urgent Staffing Solutions:

We pride ourselves in filling urgent 24/7 staffing needs for attendant care when special knowledge of injury-related care is needed. We have successfully staffed many rural 24/7 cases in as few as 72 hours. Of course – we would prefer a week or two notice, but we are good at what we do and are available when you need to find urgent, quality in home care.

It is our goal to provide an in-home care environment that is as close to “normal” as possible while encouraging progress toward pre-accident abilities.
